Changing Your Personal Details

Do we have your correct address?

It is important that we have your current home address and that you reply to any letters or communications received from either the practice or NHS England.

• If we do not have your correct address, please do contact the practice via the website to update us.

NHS England is currently writing to patients, and if the letters are returned, there is a risk that you will be removed from our practice list.  

Changing your contact details

To update details like your name, address, phone numbers or email address:

• fill out a change of contact details form
• phone or visit the surgery during our opening hours

Change of address

If you have moved to a new address, you will need to be in our catchment area to stay registered with our surgery.

Changing your phone number or email address

You will need to show us identification before we can change your phone number or email address.

Changing your date of birth or name

You will need to show us proof that you have changed your name or date of birth. This could be a marriage or civil partnership certificate, deed poll or birth certificate.
